The Gleason scoring system is the most common prostate cancer grading system used. The pathologist looks at how the cancer cells are arranged in the prostate and assigns a score on a scale of 3 to 5 from 2 different locations. Cancer cells that look similar to healthy cells receive a low score. Cancer cells that look less like healthy cells or look more aggressive receive a higher score. To assign the numbers, the pathologist determines the main pattern of cell growth, which is the area where the cancer is most obvious and looks for another area of growth. The doctor then gives each area a score from 3 to 5. The scores are added together to come up with an overall score between 6 and 10.Prostate cancer starts in the prostate gland. Castration is either by surgery or by using hormones to stop your body from making testosterone.Radical prostatectomy is the surgical removal of the prostate gland. Usually, this treatment is performed when the cancer is located only in the prostate gland. "Watch and wait," is a phrase that is being used more frequently to describe a program of active surveillance without other cancer treatment for some patients with prostate cancer. It means that if your cancer is not aggressive (based on the Gleason score and the cancer stage), treatments may be deferred and your condition periodically checked. This approach is used because the risks of urinary and sexual problems inherent in most prostate cancer treatments are serious and may be put off or avoided if the cancer is not aggressive. However, aggressive prostatic cancer is usually treated even if secondary complications of treatments are serious.Huggins C, Hodges CV. Studies on prostatic cancer. I. Prostate cancer treatments are especially concerning for men, because they often impair their ability to have sex, urinate or perform other key life functions. That’s why a treatment plan that includes side effect management is important for patients’ quality of life and overall well-being. A biopsy is done by inserting a needle through the rectum or between the rectum and scrotal junction and then removing small samples of prostatic tissue that can be examined under a microscope for cancer tissue. The biopsy may detect and determine the aggressiveness of prostatic cancer cells.

Tumor stage shows the size and spread of the cancer. Cancer in only a small part of the prostate is more treatable than cancer that has spread all through it. Tumors found only in the prostate are more successfully treated than those that have metastasized (spread) outside the prostate. Tumors that have spread to places far from the prostate like lymph nodes or bone are the most difficult to manage and have the poorest results. The treatment stops testosterone by blocking the androgen receptors in the prostate cells. Normally, testosterone would bind with these receptors. This fuels growth of prostate cancer cells. With the receptors blocked, testosterone cannot "feed" the prostate. Anti-androgen therapy does not lower testosterone. So it may have fewer or milder side effects than surgery and medical hormone treatment.
