The obturator sign or Cope's obturator test is an indicator of irritation to the obturator internus muscle.[1]. The technique for detecting the obturator sign, called the obturator test, is carried out on.. Diffusion-weighted MRI (DWI) has become increasingly valuable in the assessment of vertebral body diseases, especially in the differentiation of benign and malignant vertebral body fractures (Baur et al., 1998). In early studies, diffusion abnormalities were reported in infectious endplate changes, with no high-diffusion signal at the site of endplate abnormalities in any patients with degenerative changes (Eguchi et al., 2011). The “claw sign” (well-defined, linear regions of high signal situated within the adjacent vertebral bodies at the interface of normal with abnormal marrow) has recently been described as useful and accurate for distinguishing degenerative spondylosis from discitis-osteomyelitis in patients with Modic type 1 endplate changes (Patel et al., 2014).The appendix is susceptible to luminal obstruction via lymphoid hyperplasia, a retained fecalith, tumor, foreign body, or kink. Any of these processes may result in lymphatic and venous obstruction that increases intraluminal pressure and causes distention of the appendiceal lumen. Consequently, an acute inflammatory response develops that leads to ischemia, bacterial overgrowth, and eventually necrosis. Unless surgically removed, the gangrenous appendix will perforate, releasing the appendiceal contents into the peritoneal cavity. Subsequently, a phlegmon, intraperitoneal abscess, or local peritonitis develops. When the infection passes through the ligament, inflammatory masses extend along several segments and form epidural phlegmon or abscesses. On postcontrast T1-WI, homogeneous enhancement will be seen in the epidural phlegmon, whereas peripheral enhancement indicates an abscess formation (Fig. 35.3). Epidural abscesses associated with discitis/osteomyelitis are usually seen in the anterior part of the epidural space with extension over three to four vertebrae (Fig. 35.3).

Plain radiographs of the abdomen are generally unnecessary and contribute little to the evaluation of patients suspected to have appendicitis, unless small bowel obstruction is suspected. Some findings on abdominal radiographs that may suggest appendicitis include the presence of a fecalith, scoliosis, a focally dilated loop of bowel in the right lower quadrant (sentinel loop), focal loss of the right psoas shadow (psoas sign), and bowel obstruction.17ALL THE INFORMATION ON THIS WEBSITE IS INTENDED FOR EDUCATIONAL PURPOSES ONLY, AND SHOULD NOT BE INTERPRETED AS MEDICAL ADVICE. BMW 8-series is expected to launch in India at a price of Rs.85 to Rs. 95 lakhs ex-showroom.   Read MorePain with pressure over McBurney point, or two-thirds the distance between the umbilicus and anterior superior iliac spine, is the physical exam finding in the vast majority of patients (91%) and corresponds with an inflamed appendix lying within the typical location in the right lower quadrant.1 For patients with an atypical position of the appendix, such as retrocecal or deep pelvic, pressure in this location may not elicit the typical exam findings. In the case of a retrocecal appendix the pain may be localized over the right flank, elicited by an extension of the hip (psoas sign), or may not be well localized on physical exam at all. An obturator sign may be present with appendicitis located in the deep pelvis. A Rosving sign consists of pain in the right lower quadrant with palpation in the left lower quadrant. Jostling the bed or striking the heel of the extended right leg can lead to right lower quadrant pain. Similarly the patient may be asked to bounce or attempt to jump, producing pain in the area. Although these findings are typical for an uncomplicated appendicitis, perforation can lead to a more generalized peritonitis, with involuntary diffuse guarding, high fever, and hemodynamic changes. Psoas sign: Pain on passive extension of the right thigh. It is present when the inflamed appendix is retrocecal and overlying the right psoas muscle. Two routes of infection are possible: hematogenous (arterial and venous) and nonhematogenous (Tali et al., 2015). The venous route of infection to the spine is through the valveless epidural venous plexus. Nonhematogenous spread may occur after penetrating trauma, as well as surgical and interventional procedures. In children <2 years of age, the most common symptoms are vomiting, abdominal pain, fever, abdominal distention, diarrhea, irritability, right hip pain, and limp.15 In children 2 to 5 years of age, abdominal pain precedes vomiting and usually is associated with fever and anorexia. Tenesmus, which can be perceived as diarrhea, is common in infants and toddlers and may lead to misdiagnosis.7 School-aged children describe abdominal pain that is constant and worse with movement or coughing.15 Nausea, vomiting, anorexia, tenesmus, and dysuria also occur in this age group.15 Older children sometimes report that they are hungry.1 A few studies have attempted to determine the sensitivity and specificity of these symptoms. Two retrospective studies of children21,22 and one prospective study of children and adults23 found that vomiting has a low sensitivity (0.43) and specificity (0.64).24 Fever has better sensitivity and specificity: 0.75 and 0.78, respectively. Anorexia and pain migration have low sensitivities (0.64 for anorexia, 0.41 for pain migration) and specificities (0.43 for anorexia, 0.54 for pain migration).24 In older children the most sensitive symptom is increased pain with coughing and movement (0.8), but the specificity is low (0.52).24

In the past, plain abdominal radiographs were the most common imaging study performed in patients with suspected appendicitis. However, plain abdominal radiographs have been found to be normal or misleading in up to 82% of children with appendicitis.15 Although the presence of a fecalith is suggestive of appendicitis,36 fecaliths occur with low frequency.24 Fecal loading in the cecum appears to be a sensitive and specific radiographic finding for appendicitis in adult and pediatric patients.37FPnotebook.com is a rapid access, point-of-care medical reference for primary care and emergency clinicians. In adults with appendicitis, computed tomography (CT) has higher specificity and sensitivity than US.47 Studies in children also have shown high sensitivity (94% to 99%) and specificity (87% to 99%) of CT for the diagnosis of appendicitis.40,47,48 Several of these studies have been prospective, although not blinded. Limitations of CT include: cost, the need for oral contrast and possible sedation, and exposure to ionizing radiation.49Appendicitis can mimic many disorders (Box 68-2). Other inflammatory diseases, most commonly gastroenteritis, can produce signs and symptoms similar to appendicitis.1,27 In girls, ovarian pathology such as ruptured ovarian cyst and ovarian torsion can cause right lower quadrant pain and peritoneal irritation. In neutropenic patients, differentiating appendicitis from typhlitis or neutropenic enterocolitis can be difficult and imaging studies often are necessary.1,28

Diagnostic criteria include: appendiceal diameter >6 mm, a target sign with 5 concentric layers, distention or obstruction of the appendiceal lumen, high echogenicity surrounding the appendix, an appendicolith, fluid surrounding the appendix, enlarged and thickened bowel wall and lack of peristalsis.24 Sensitivity of US ranges from 71% to 92% and specificity from 96% to 98%.38–41 Patient characteristics that limit US utility include obesity and gaseous distention of bowel loops.42 Value of US also is dependent on the experience of the ultrasonographer. Many authors recommend US as the first study for children with suspected appendicitis due to its low cost, freedom from ionizing radiation, and its superiority to clinical judgment in equivocal cases.43–46Findings having little or no diagnostic value in diagnosing appendicitis are rectal tenderness (whether the tenderness is generalized or confined to the right rectum) and the obturator sign (LRs not significant).50 Nonetheless, a rectal examination should still be performed to detect the rare patient (2% or less) with a pelvic abscess and rectal mass.30,32 The obturator sign or Cope's obturator test is an indicator of irritation to the obturator internus 23.10.2019 · The Obturator sign involves a doctor manipulating a patient's right leg inward to slightly.. Its use has been tested in children and renamed the Pediatric Appendicitis Score (PAS). Although some authors have found the scores to be accurate,57 most studies in young children show only moderate sensitivity (76% to 90%) and specificity (50% to 81%) when the score is ≥7. In older children (over 16 years), a score of ≥7 has a sensitivity of 100% and a specificity of 93%.24,58 Another study found the PAS most useful when ≤4, to exclude appendicitis (without need for imaging), and ≥8, to confirm appendicitis (without imaging).59

A prospective comparison of whole-body 18F-FDG PET/computed tomography (CT) and MRI of the spine in the diagnosis of hematogenous spondylodiscitis reportedly showed that the combination of 18F-FDG PET/CT and MRI increases the rate of infection detection to 100% (Fuster et al., 2015). 18F-FDG uptake, quantified in terms of maximum standardized uptake value (SUVmax) corrected by the background SUVmean, was significantly higher in patients with spondylodiscitis than in those without infection (Fuster et al., 2015) (Fig. 35.2).

Typical MRI findings include narrowing of the affected disc space, with irregularities and signal changes of the affected vertebral body endplates (T1 low, T2 high signal intensity), and strong enhancement on postcontrast T1-WI images (Fig. 35.1). The involved disc shows T2 high signal (hot disc) with a loss of the intranuclear cleft and peripheral enhancement (Fig. 35.1). Increased T2 signal of the disc is not specific to pyogenic spondylitis, and has also been described in highly vascularized degenerative discs in erosive intervertebral osteochondritis (Stäbler and Reiser, 2001). Diabetes mellitus, ethanol abuse, renal and hepatic failure, malignancy, and immunosuppression are conditions associated with pyogenic spondylodiscitis in the elderly population. The clinical and laboratory findings of bacterial spondylodiscitis can be subtle and unspecific. Spondylodiscitis is usually suspected in a patient with back pain and fever. Final diagnosis requires biopsy, which may occasionally be nondiagnostic. Intravenous drug abuse is the most common cause in younger patients. The most isolated agent responsible for pyogenic spondylodiscitis is Staphylococcus aureus (32–67%), followed by Streptococcus species (Mylona et al., 2008). Pseudomonas aeruginosa is reported to be common in intravenous drug abusers (Mylona et al., 2008). Infection with Staphylococcus epidermidis is associated with spine procedures, Streptococcus and Enterococcus with infective endocarditis, Escherichia coli with urinary tract infection, Salmonella with sickle cell disease, and anaerobic bacteria will be isolated in abdominal and pelvic infections and gastrointestinal procedures. Maximal pinpoint tenderness over this “sonographic McBurney point” had superior diagnostic accuracy for detecting appendicitis (sensitivity = 87%, specificity = 90%, positive LR = 8.4, negative LR = 0.1).83Physical findings also vary by patient age. In those <2 years of age, nonspecific signs such as fever and diffuse tenderness are the most common findings. Preschool children aged 2 to 5 years demonstrate right lower quadrant tenderness, fever, and involuntary guarding. School-aged children are more likely to have localized right lower quadrant tenderness, or diffuse guarding and rebound tenderness, if the appendix has perforated.24 Two studies25,26 have examined rebound tenderness in children and found low sensitivity (50%) and specificity (60%). The sensitivity and specificity of signs equated with appendicitis in adults, including Rovsing sign (pressure palpation in left lower quadrant induces pain at McBurney point), obturator sign, and psoas sign, have not been critically evaluated in children.24 Obturator Sign - Physical Exam OBTURATOR SIGN: Obturator test is performed when the physical suspects the patient may be presenting with acute appendicitis. The false-negative rate of an imaging study is increased if the study is performed in a child with a high clinical suspicion of disease; the false-positive rate is increased if the study is performed in a child with a low clinical suspicion of disease.50 Imaging studies are most useful when used only in children with clinically equivocal presentations. Further indications for CT scan may be in the evaluation of children with suspected complicated appendicitis, as this diagnosis can influence the management strategy, directing the surgeon toward initial nonoperative therapy.51 In one study, an approach based on selective imaging for equivocal cases, using US, followed by CT for inconclusive studies, has been shown to be accurate and cost-effective,50,52 with sensitivity 99%, specificity 92%, and accuracy 97%.53Rectal tenderness (see EBM Box 52.1) and the obturator sign (see EBM Box 52.2) were diagnostically unhelpful in these studies.
